Good day. We are making an investigation, a worker died last fall... I am trying to find the "main problem", and reduce the number of accident on bulk carriers. From your graphic, they increased a lot in the last 20 years.

Do you know if most of these accidents were on ships with "Australian ladders" ? I want to suggest improvement on this design, expect that small changes can save lives...

Australian ladders are much safer than straight vertical ladders. However, it seems that we have more accident as we increase the number of Australian ladders on ships...
