All ITF affiliates attending the Durban Congress are being asked to complete this form  online (or in paper form and to return them to us before you leave).

The ITF needs much more information on union membership in key companies and operations. The form is simple and will not take much time to fill in. It is available in English, French* and Spanish* (pending translations).

This form has identified a number of specific companies and operations. There are of course many other companies and operations which we shall need to map in due time. This is the start of an ongoing mapping process, which aims to assist the ITF to take a more strategic approach to its global programme of work.

Please complete the parts of the form, which are relevant to your union.

Some guidance.

Most unions have several members on their delegation - Only one completed survey is needed per union.

• Please make sure that at least one member of the delegation completes the form.
  
  
• If in doubt complete the form yourself,  it will not matter if we receive more than one form from your union.
  
  
• Please answer all relevant questions.
  
  
• If you do not organize in a specific company or operation just put NO in the box. This information is also useful.
  
  
• If you do not know something for sure please just put down your best guess.
  
  
• Please remember to put your name and email address on the form.
  

During the Congress ITF staff or volunteers may approach you to find out if you have completed and returned this form. Please be as helpful to them as possible.
